The quick answer

|---|---|---|
| New two-year card | $233.30 | $151.50 |
| Two-year renewal | $203.30 | $121.50 |
These totals include our physician evaluation and the OMMA application fee with card processing. The reduced state fee applies to patients enrolled in Medicaid (SoonerSelect) or Medicare, and to veterans with 100% disability status.
The two fees, explained
Getting an Oklahoma medical marijuana card always involves two separate payments to two different organizations.
1. The physician evaluation (paid to us)
Oklahoma requires a recommendation from an Oklahoma-licensed physician who is registered with OMMA. Our evaluation is $129 for new patients and $99 for renewals. That covers your video or phone visit, the physician's review of your history and medications, your signed Adult Patient Physician Recommendation Form, an OMMA application checklist, and patient support until your card arrives. There are no membership fees, and we don't charge extra for paperwork help.
2. The state application fee (paid to OMMA)
The Oklahoma Medical Marijuana Authority charges $100 for a patient license, plus a $4.30 card processing fee, for a total of $104.30. With the reduced fee, it's $20 plus $2.50 in processing, or $22.50. You pay this directly to the state when you apply in the OMMA MedPortal. We never collect it.

Cost by license type
| License | Our evaluation | OMMA fee | Total |
|---|---|---|---|
| Adult patient, new (2 years) | $129 | $104.30 | $233.30 |
| Adult patient, new, reduced fee | $129 | $22.50 | $151.50 |
| Adult patient renewal | $99 | $104.30 | $203.30 |
| Adult patient renewal, reduced fee | $99 | $22.50 | $121.50 |
| Short-term adult (60 days) | $129 | $104.30 | $233.30 |
| Minor patient (2 years) | Contact us* | $104.30 or $22.50 | Varies |
| Out-of-state temporary (30 days) | Not needed | $104.30 | $104.30 |
| Caregiver | Not needed | No charge | $0 |
| Replacement card | Not needed | $22.50 | $22.50 |
*Minor patients need recommendations from two physicians. Contact us before booking so we can explain the process and pricing. Out-of-state applicants use their home state's medical marijuana license instead of an Oklahoma physician recommendation, and don't qualify for the reduced state fee.
When you pay, step by step
- At booking: we place a temporary authorization hold of $129 (or $99). It's not a charge.
- After your visit: if your physician approves and signs your recommendation, we charge the held amount. If not, we release the hold and you pay nothing.
- When you apply to OMMA: you pay the state fee by Visa, Mastercard or Discover in the MedPortal. Submit within 30 days of your recommendation's signature date.
Protect your state fee. OMMA doesn't refund application fees, even when an application is denied. Most denials come from document problems: an out-of-date utility bill, a blurry ID, or a photo that doesn't meet requirements. Use our checklist before you submit. If OMMA denies you for a document issue, we'll help you fix it and reissue your recommendation at no charge within 60 days of your visit if it has expired.
Who qualifies for the reduced state fee
OMMA lowers its fee from $100 to $20 for Oklahoma patients who upload acceptable proof of one of the following:
- Medicaid (SoonerSelect) enrollment: your Medicaid card plus a screenshot or download of your eligibility page from mysoonercare.org showing your coverage start and end dates. OMMA requires both.
- Medicare enrollment, such as your Medicare card
- 100% disabled veteran status, such as a VA benefits letter showing a 100% service-connected disability rating
The reduced fee saves $81.80 on each two-year license. It applies to new cards and renewals, but not to out-of-state temporary licenses. Our evaluation fee is the same for every patient.
What it costs over two years
| Item | Total | Per month | Per week |
|---|---|---|---|
| New card, standard | $233.30 | $9.72 | $2.24 |
| New card, reduced fee | $151.50 | $6.31 | $1.46 |
| Renewal, standard | $203.30 | $8.47 | $1.95 |
| Renewal, reduced fee | $121.50 | $5.06 | $1.17 |
A license lasts up to two years, so the real cost is less than $10 a month. Renewing before your card expires also avoids a gap in legal protection.

The cost of medicine
Your card lets you buy from licensed Oklahoma dispensaries. Medical marijuana purchases carry a 7% state excise tax plus state and local sales tax. Prices vary widely by product, potency and location, and Oklahoma has one of the most competitive dispensary markets in the country.
Licensed patients may also grow up to six mature plants and six seedlings at home, which some patients use to lower their long-term costs. Our physicians can talk with you about product types and starting doses so you don't spend money on products that aren't a good fit.
Comparing providers
Evaluation prices in Oklahoma vary. When you compare, look at the full cost and what you're actually getting:
- Is the physician registered with OMMA? Since January 1, 2026, OMMA rejects recommendations from unregistered physicians under SB 1066. A rejected application costs you the non-refundable $104.30 state fee.
- When are you charged? Some services charge upfront and make you request a refund if you aren't approved.
- Are there add-on fees? Look for memberships, "application assistance" charges, rush fees or document fees.
- Is it a real medical visit? A review of your medications and risks has value beyond the form.
Ways to save
- Claim the reduced state fee if you're enrolled in Medicaid or Medicare, or are a 100% disabled veteran.
- Get your documents right the first time so you don't pay the state fee twice.
- Book your renewal 30 to 45 days early to keep the $99 renewal rate and avoid a lapse.
- Keep your card safe. A replacement costs $22.50.
- If you have a caregiver, their license is free.
Cost questions
How much does an Oklahoma medical marijuana card cost in total?
For a new two-year adult card, most patients pay $233.30: $129 for our physician evaluation plus $104.30 to OMMA ($100 application fee and a $4.30 card processing fee). With the reduced state fee for Medicaid, Medicare or 100% disabled veteran patients, the total is $151.50.
How much is a renewal?
Renewals are $99 with us plus the same OMMA fee, for a total of $203.30, or $121.50 with the reduced state fee. OMMA does not discount renewals.
When do I pay each fee?
You pay our evaluation fee through our booking system, as explained above. You pay the OMMA fee yourself, by Visa, Mastercard or Discover, when you submit your application in the OMMA MedPortal.
Is the state fee refundable if OMMA denies my application?
No. OMMA states that application and processing fees are non-refundable, including for applications that aren't approved. That's why it's worth double-checking your residency documents, ID and photo before you submit.
Does insurance cover a medical marijuana card?
No. Health insurance, Medicaid and Medicare don't pay for medical marijuana evaluations, state license fees or cannabis products, because marijuana remains illegal under federal law. Medicaid and Medicare enrollment does qualify you for the reduced $20 state fee.
Can I use my HSA or FSA?
Cannabis products are generally not eligible HSA or FSA expenses because marijuana is federally illegal. Ask your plan administrator about the physician evaluation specifically, since rules vary by plan.
How much does a replacement card cost?
OMMA charges $20 plus a $2.50 processing fee ($22.50) to replace a lost or damaged card. You don't need a new physician recommendation or a visit with us.
Are there cheaper options?
Some services advertise lower evaluation prices, but compare the full cost. Watch for membership fees, separate charges for paperwork help, and physicians who aren't registered with OMMA under SB 1066. A recommendation from an unregistered physician is rejected, and you lose the $104.30 state fee.
